These services reflect the full range of support we provide. Availability depends on the client’s needs, the assessment and the scope of our Minnesota Comprehensive Home Care License.

Built around you

One agency, the full range of support

Whether it is a few hours of companionship a week or clinical support delegated by a nurse, your care plan pulls from the same list. Nothing is bolted on from a different provider, and nothing is bundled in that you did not ask for.

Services fall into three groups: the everyday personal support most clients need, the nursing and clinical care that is arranged after assessment, and the planning and coordination work that holds the whole plan together.

Depending on eligibility and authorization, services may also be delivered through applicable Home and Community-Based Services (HCBS) programs.

Group 1 of 3

Personal & Daily Living Support

Non-clinical, hands-on assistance with the everyday tasks that keep a person safe, clean and comfortable in their own home.

Personal Care Assistance

One-on-one help with the daily routines that support health, hygiene and dignity.

Our caregivers provide hands-on assistance with activities of daily living, guided by an individualized care plan and reviewed on an ongoing basis.

Bathing & Grooming Assistance

Respectful help with bathing, oral care, hair and skin care.

We support bathing and grooming routines with patience and privacy, reducing fall risk while preserving independence wherever possible.

Dressing Assistance

Support choosing and putting on clothing safely and comfortably.

Caregivers assist with dressing and undressing, including adaptive clothing needs, at a pace that respects the client's comfort.

Mobility & Transfer Assistance

Safe help moving around the home, in and out of bed, and with walking aids.

We assist with walking, transfers and positioning to reduce fall risk, using proper technique and any mobility equipment already in place.

Meal Preparation

Nutritious meals prepared in the home according to dietary needs.

Caregivers plan and prepare meals and snacks that reflect dietary restrictions, preferences and physician guidance.

Light Housekeeping

A clean, safe living space — laundry, tidying and basic upkeep.

Light housekeeping keeps the home safe and comfortable: laundry, dishes, tidying, and general upkeep of client living areas.

Companionship Care

Friendly, consistent company and conversation that guards against isolation.

Companionship visits provide conversation, shared activities and a familiar, trusted presence for clients who live alone or spend long hours without company.

Respite Care

Scheduled relief for family caregivers, without a gap in care.

We step in for a few hours, a day, or longer so family caregivers can rest, work or attend to their own needs, with continuity for the client.

Appointment & Errand Assistance

Support with appointments, community outings and essential errands.

Caregivers can accompany clients to appointments and errands, providing support getting to, from and through the visit when this is part of the agreed care plan.

Medication Reminders

Non-clinical cueing to help clients stay on their own medication schedule.

For clients who self-administer medication, caregivers provide reminders and observation to support an existing medication routine.

Nursing & Clinical Care

Clinical support is discussed during assessment and provided only when appropriate to the client’s needs and within our licensed scope.

Nursing Assessment & Oversight

Assessment and care planning for clients with clinical needs.

Care planning and clinical support are coordinated with the client’s care team when appropriate and within the scope of the services agreed for that client.

Medication Management

Structured medication set-up, administration support and monitoring.

Medication support may include reminders, administration support and monitoring when appropriate, based on the client’s plan and applicable requirements.

Wound Care

Assessment, cleaning, dressing and monitoring within the approved care plan.

Wound support focuses on healing and infection prevention for people recovering from surgery or injury, or living with a chronic wound, and is provided as appropriate to the client’s condition and within our licensed scope.

Vaccinations

Vaccine support delivered in a comfortable, professional setting.

Access to seasonal and routine vaccinations, which matter most for seniors and people who are medically vulnerable. Availability and follow-up depend on clinical appropriateness, current requirements and the client’s care plan.

Basic Nursing Care

Nursing support and physical assistance in a non-hospital setting.

For people who need daily attention rather than a hospital bed: monitoring vital signs and general health, and assistance with mobility, hygiene, dressing and nutrition when included in the client’s plan.

Respiratory Care

Breathing support and monitoring as part of an established plan of care.

Support for people living with ongoing respiratory needs at home, coordinated with the client’s care team and provided within our licensed scope.

Ventilator Management

Support for people who require respiratory assistance at home.

Ventilator support is provided for clients whose plan of care calls for it, with caregiving matched to the level of clinical need identified at assessment.

Tracheostomy Care

Routine tracheostomy support within the approved plan of care.

Airway support delivered in the home, coordinated with the client’s clinicians and provided only where it is appropriate and within our licensed scope.

Gastrointestinal Care

Support for feeding, nutrition and digestive health needs.

Assistance with the routines and equipment that support nutrition and digestive health at home, as set out in the client’s care plan.

Neurological Care

Day-to-day support for people living with a neurological condition.

Consistency matters here. Care is planned around the routines, cues and safety needs of the individual and revised as the condition changes.

Private-Duty & Home Care Nursing

Skilled nursing support in the comfort of the client’s own home.

One-to-one nursing hours for people whose needs go beyond personal care, provided within our licensed scope and coordinated with the wider care team.

Home Health Aide & PCA Support

Trained aide and personal care assistant hours as part of the plan.

Aide and PCA support covers hands-on help with activities of daily living, delivered by staff matched to the level of support the assessment identifies.

Care Planning & Coordination

The work around the care itself — assessment, planning, goal setting, education and the communication that keeps everyone involved pointing the same way.

Assessments & Evaluations

A structured look at what is actually needed before anything is agreed.

The assessment establishes the appropriate level of intervention and is the basis for every other decision in the plan. Nothing is quoted or scheduled before it.

Care Plan Development

A plan shaped around medical history, lifestyle, preferences and goals.

Care plans are developed with the client and, where appropriate, family members and healthcare providers, then reviewed and revised as needs change.

Care Coordination

Organized communication across appointments, referrals and providers.

Managing care across several providers is where families lose the most time. Coordination covers appointment planning, referrals, medication updates and communication with the client’s care team.

Clinical Goals Development

Individual goals for health, mobility, nutrition, medication stability or well-being.

Goals give the plan something to measure against. They are set with the client and their care team and included only when appropriate to that person’s needs and agreed services.

Patient Education

Clear education for clients and families about conditions, medications and routines.

Education is adapted to the client’s needs, learning style and language, and supports informed decisions, self-advocacy and safer care at home.

Mental Health Support

Attention to emotional well-being alongside physical care.

Isolation and low mood affect recovery as much as any physical symptom. Support here is built into the plan rather than treated as an extra.

Continuing Care

Support that stays in place as needs change over time.

Care rarely stays still. Continuing care means the plan is reviewed on an ongoing basis and adjusted rather than restarted every time circumstances shift.

Referral Assistance for Legal Housing

Help locating housing resources and the support services that go with them.

Stable housing underpins everything else. We can help identify resources, complete applications and connect clients with housing partners and community programs; placement itself is not guaranteed.

Clinical support is discussed at assessment and provided only where it is appropriate and within our licensed scope. If you need something outside that scope, we will say so and help you find the right provider rather than stretch a care plan to fit.
